Support Service Providers

If you are HIV positive, live in Contra Costa County, and are interested in accessing any of these services please contact Contra Costa County HIV/AIDS & STD program at 925-608-5384 and speak to a Medical Social Worker of the day who will help and determine your eligibility for services. After you are enrolled your medical case manager will refer you to these services as needed.

To access the following services, a referral from a Medical Social Worker is required.
