Chronic Disease Prevention

The overall mission of CW&PP's approach to chronic disease prevention is for community residents, local government, community organizations, the Health Services Department (HSD) and others to work together to create a healthier community environment, particularly in high-risk communities. It aims to increase the ability of these communities to reduce chronic disease incidence and death by reducing their exposure to key risk factors.

This will be accomplished by: (1) increasing and enhancing the environmental conditions (such as advertisements and media messages) that promote good health; (2) reducing the number and impact of environmental conditions that promote poor health; and (3) creating and strengthening community norms that support health-promoting behaviors through collaborative planning with communities, specific educational programs, media campaigns, policy and advocacy efforts, and improved economic development.
